SouthEast Metro SAIL / Eldercare Development Partnership

SouthEast Metro Seniors Agenda for Independent Living (SAIL) is an Elder Care Development Partnership of seniors and senior service providers serving the counties of Dakota, Ramsey and Scott. The mission of SouthEast Metro SAIL is to reshape the long term-care system in the Southeast Metro area, making more community options available and accessible to older persons.

SouthEast Metro SAIL is not a direct service organization. It works towards its mission by bringing together consumers and providers of senior services interested in participating in resource development and systems change. These strategies are designed to create easier access to senior services and develop new and/or more efficient senior service options. Current strategies are looking at ways to:

  • Work with Counties and other long-term care service providers to collaboratively develop sustainable proposals for systems change, accessing the State's CS/SD grant funds and other resources as appropriate.
  • Educate care managers, clients, and their families on the availability, functional use, and reimbursement options for new technologies that support seniors living in their own homes.
  • Foster the development of additional home care resources in the Belle Plain/Jordan area of Scott County.
  • Develop a new model of consumer directed community supports within the Alternative Care Program.
  • Facilitate discussions between counties and Minnesota Senior Health Option health plans that address and resolve the issues arising from MSHO passive enrollment and identify best practices in care coordination.
  • Participate in the development, scheduling and delivery of Early Dementia Identification training sessions for new county and care system care managers.
  • Facilitate discussions and provide technical assistance to one or more of the communities in each of the counties of the SouthEast Metro SAIL region that provides information, tools, and models on livable communities that can be adapted to the local community.
