Title III-D Medication Therapy Management Service

Fairview Pharmacy Services Pharmaceutical Care for Underserved Elderly

Pharmaceutical Care is a group of services that optimize medication therapeutic outcomes for individual seniors. The service is provided by licensed pharmacists, in the home or other location as determined by the individual elders and is available in the seven-county Metropolitan area (Anoka, Carver, Dakota, Hennepin, Ramsey, Scott and Washington). The target populations are seniors who:

  • have multiple medications (4+ medications including over-the-counter drugs) and has no other identified resources available to assist them with medication therapy management;

  • are determined to be frail and homebound by reason of illness, disability or functional need, or otherwise isolated;

  • have the greatest economic and social need with particular attention to being low-income and a minority.

Contact Fairview Pharmacy Services at 612-538-0026 to learn more. FPS will contact the seniors and/or caregivers within 1-3 business days to arrange for a home visit by a licensed pharmacist. FPS’s pharmaceutical care includes:

  • Initial face-to-face medication assessment;

  • Therapy management;

  • Follow-up(s);

  • Evaluation to document outcomes; and 

  • Coordination of care/service referrals
