Two fliers in one file. The first flier explains the different
community based nurses including faith based (parish), homecare,
public health and living at home / block nurses. The second flier
explains various faith based programs available for seniors including
faith based (parish) nurses, health / care team ministries,
Befrienders, Faith in Action and Stephen's Ministries.
Faith Community
Nurse Network of the Twin Cities (Off Site)
The mission of the FCNN is to champion faith community nursing in
the Twin Cities metropolitan area through education, networking,
resource and program development, contributions to research, and
leadership. Our efforts are focused on building and sustaining effective
faith community nursing programs in congregations of all faith
traditions and, in this way, bridging the gaps between the faith and
health communities.
Parish Nurse Effectiveness Study (pdf)
The Metropolitan Area Agency on Aging received funds through the
Community Service/Services Development grant program from the
Minnesota Department of Human Services to analyze the interventions of
parish nurses on older persons and discern their impact on health care
costs. Using the DIARY methodology 75 parish nurses documented
activities that often resulted in preventing the need for expensive
acute or long-term health care while promoting independence and
emotional well-being.
Care Team Ministry
Replication Module (pdf)
Developed by the Normandale Ministry for Healing & Wholeness as a part
of the Supporting Seniors Across Systems project, the module helps
faith communities develop care team ministries.
